The BBL 360 vs BBL question is one of the first things patients ask in consultation, and the answer depends far more on your body than on the name of the procedure. Both operations use your own fat to add volume and shape to the buttocks. The difference lies in how much of your trunk is sculpted to create that shape. This guide, written by our lead plastic surgeon, explains what each procedure involves, who suits which, and how cost and recovery compare.
What Is a Standard BBL?
A standard Brazilian Butt Lift harvests fat from one or two donor areas, most commonly the abdomen and flanks, purifies it and injects it into the buttocks and sometimes the hips. The goal is added projection and a rounder shape. Liposuction is targeted rather than comprehensive, so the waist and back are only treated where fat is needed for the graft. It is the right choice when you are happy with your overall silhouette and simply want a fuller bottom.
What Is a BBL 360?
A BBL 360 combines fat transfer with circumferential liposuction of the entire midsection: upper and lower abdomen, flanks, bra-line and lower back. By removing fat all the way around the trunk, the surgeon creates an hourglass contour that makes the buttocks look more prominent even before the graft is added. The harvested fat is then transferred to the buttocks and hips in the same session. It is the most requested procedure at BBL Turkey for exactly this reason.

Body Type and BMI: The Real Deciding Factor
When I assess a patient for BBL 360 vs BBL, I look at three things: how much fat is available, where it sits, and what shape they want.
Slim patients (BMI under 22)
If you have little excess fat, a full 360 liposuction would leave the surgeon chasing fat that is not there. A skinny BBL is designed for exactly this situation: fat is harvested carefully from multiple small areas, including the inner thighs, knees and arms, and transferred with a focus on shape rather than dramatic size. The result is subtle, athletic and natural.
Average build (BMI 22–27)
Most patients in this range are good candidates for either procedure. If your waist is already defined and you mainly want projection, a standard BBL is sufficient. If you carry fat around the flanks and lower back and want a visibly smaller waist, the 360 approach delivers a far more dramatic transformation for a modest increase in cost and recovery.
Fuller figure (BMI 27–32)
Here the BBL 360 is almost always the better option. There is ample fat to harvest, and sculpting the whole trunk is what creates the waist-to-hip ratio patients are looking for. Transferring fat without contouring the surrounding area can make the lower body look heavier rather than curvier.
BMI above 32
We generally ask patients to reduce their BMI to 32 or below before surgery. Higher BMIs increase anaesthetic and clotting risks and make skin retraction less predictable. Your coordinator can arrange a surgeon's review of your photos to confirm where you stand.
BBL 360 vs BBL: Cost Difference
Because a BBL 360 involves more liposuction time and more theatre resources, it costs more than a standard BBL. In Istanbul the gap is small, typically a few hundred dollars, because the hospital, anaesthesia, hotel and aftercare are already included in both packages. In the UK the difference is often £3,000–£5,000, as surgeons charge per liposuction area. For many patients this makes the 360 the better-value choice when travelling to Turkey.
BBL 360 vs BBL: Recovery Difference
The fat-transfer side of recovery is identical: no direct sitting for two to three weeks, a BBL pillow, and sleeping on your stomach. The difference is in the liposuction areas. After a BBL 360, you will have more swelling and bruising across the back and flanks, you will wear your garment for the full eight weeks, and you should allow an extra week before returning to desk work. Lymphatic massage is particularly valuable after a 360. Our full BBL recovery timeline covers each stage in detail.
Who Should Choose Which?
Choose a standard BBL if you:
- Already have a defined waist and want more projection.
- Have fat concentrated in one or two areas.
- Want a shorter operation and a slightly quicker return to work.
- Have had liposuction of the trunk previously.
Choose a BBL 360 if you:
- Want an hourglass shape, not just a fuller bottom.
- Carry fat around the abdomen, flanks or lower back.
- Have a BMI between 24 and 32.
- Want the most noticeable transformation in a single surgery.
Consider a skinny BBL if you:
- Have a BMI under 22 and limited donor fat.
- Prefer a subtle, natural enhancement.
Patients with pronounced hip dips may also want to look at our BBL with hip dip correction, which can be added to either approach. The International Society of Aesthetic Plastic Surgery (ISAPS) publishes patient safety guidance on fat grafting to the buttocks that is worth reading before any consultation, whichever procedure you choose.
Key Takeaways
- In the BBL 360 vs BBL decision, the amount and location of your fat matters more than the name.
- A standard BBL targets one or two donor areas; a BBL 360 sculpts the whole trunk for an hourglass result.
- A BBL 360 suits BMIs of 24–32; slim patients under 22 are better served by a skinny BBL.
- In Turkey the cost gap is a few hundred dollars; recovery after a 360 takes roughly one extra week.
- Both procedures share the same no-sitting rules for the first two to three weeks.
