What is a BBL revision?
A BBL revision is a second operation that corrects problems left by an earlier Brazilian Butt Lift. It is not a repeat of the first surgery. The goal is to fix what went wrong, not simply to add more fat.
Some patients want more volume because much of the fat did not survive. Others have too much fat, an uneven shape or hard lumps. Many have a mix of these issues. A revision BBL plan is always built around the problem in front of the surgeon.
Your operation is planned and performed by one of the specialist plastic, reconstructive and aesthetic surgeons on our surgical team in Istanbul. You can read each surgeon's training and qualifications on their profile, and you meet your surgeon on a video call before you book and in person before surgery. Some cases are better left alone, and he will say so.
Why a first BBL sometimes needs correcting
Fat grafting is a living-tissue procedure. Not all of the fat survives the move. The body absorbs part of it over the first months, and the amount varies from person to person. This is why two patients with the same surgery can end up with different results.
Other causes include fat placed in the wrong area, too much fat in one spot, poor liposuction technique or weight change after surgery. Sometimes the first result was fine, but life events such as pregnancy or weight loss changed the shape later. A failed BBL can mean any of these things.
Revision BBL vs primary BBL: the key differences
A revision is harder than a first BBL. The tissue has been operated on before. There may be scar tissue, less donor fat and areas where fat has died. These factors make results less predictable.
| Factor | Primary BBL | BBL revision |
| Main goal | Add volume and shape | Correct shape, balance and texture |
| Tissue | Untouched tissue | Scar tissue and old grafts |
| Donor fat | Usually plentiful | Often reduced after the first liposuction |
| Surgery time | 3–4 hours | 4–6 hours |
| Planning | Photos and exam | Photos, exam, ultrasound and past records |
| Predictability | Good | Lower; results vary more |

What are the signs you may need a BBL revision?
Not every unhappy feeling after a BBL means you need more surgery. But some signs are worth a specialist review:
- One side looks clearly larger, higher or wider than the other a year after surgery
- The shape has gone flat and looks close to how it did before
- There is a hard ledge on the upper buttock, or the shape looks square
- You can see or feel lumps, dents or ridges in the buttock or donor areas
- There is a dip or sharp step between the waist, hips and buttock
- Firm lumps are painful, growing or changing
If you notice any of these, send photos for a free review. Our surgeon will tell you whether a BBL revision makes sense, or whether waiting is the better choice.
What problems can a BBL revision fix?
A BBL revision can improve most common shape problems after a first BBL. These include uneven sides, lost volume, a hard ledge, too much fat, a flat hip line, lumps and scarring. It cannot always make the result perfect.
Asymmetry between the two sides
One side may look bigger, higher or wider than the other. This can come from uneven fat placement or from uneven fat survival. The surgeon may reduce the larger side, add fat to the smaller side, or do both. Perfect symmetry is not possible, because no body is fully symmetrical.
Volume loss and fat resorption
Some patients lose much of their new volume in the first six months. The result can look flat or close to how it looked before surgery. If you have enough donor fat, a revision can add volume back with a careful re-graft. Fat survival after revision can be lower than the first time, because scar tissue has a weaker blood supply.
The "shelf" or square look
A "shelf" is a ledge where too much fat sits high on the upper buttock. A square look happens when the shape lacks a curve at the sides or bottom. These shapes are improved by reducing fat in the high areas and adding it lower or to the sides. The aim is a smooth line from the waist into the buttock.
Overfilling
Too much fat can look unnatural and can feel heavy. Overfilled areas can also be firmer and more prone to lumps. Your surgeon can remove some of the fat with gentle liposuction. The aim is a balanced shape that suits your frame.
Poor hip transition and hip dips
Some first BBLs add volume to the buttock but ignore the hips. This leaves a dip or a sharp step at the side. A revision can add fat to the upper hip and refine the waist so the curve flows. Our BBL with hip dip correction page explains this area in more detail.
Contour irregularities
Dents, ridges and a wavy surface often come from uneven liposuction in the donor areas. They can also appear on the buttock where fat has clumped. These are treated by smoothing the high spots with fine cannulas and, where needed, adding small amounts of fat to the low spots.
Fat necrosis and oil cysts
When grafted fat dies, it can form firm lumps (fat necrosis) or pockets of liquid oil (oil cysts). Some are painless and small. Others are tender or visible. Ultrasound shows their size and depth. Larger or painful cysts can be drained or removed during the BBL revision.
Scarring and tethering
Scar bands under the skin can pull the surface down and cause dimples. They can also make it hard for new fat to spread evenly. A technique called subcision releases these bands. Visible entry scars from the first surgery can sometimes be revised at the same time.
Who is a good candidate for a revision BBL?
You may be a good candidate for a revision BBL if your first BBL was at least 12 months ago, you have a clear concern and you are in good health. You also need enough donor fat if volume must be added, and realistic goals.
Why you should wait at least 12 months
Swelling can last for many months after a BBL. Fat keeps settling, and scar tissue keeps softening, for up to a year. If you operate too early, the surgeon may correct a problem that would have improved on its own. Waiting 12 months gives a stable base to plan from. In some cases, such as a painful cyst or infection, earlier treatment may be needed. This is decided case by case.
Enough donor fat
Many revision patients have already had liposuction of the stomach, flanks and back. There may be less fat left to use. Possible donor areas include the inner and outer thighs, arms, the upper back or areas that were treated lightly the first time. If donor fat is very limited, the plan may focus on reshaping rather than adding size.
Some slim patients are advised to gain a small amount of weight before surgery. Others may be better suited to a staged plan. Our Skinny BBL page explains how we work with patients who have little fat.
Imaging of your previous grafts
Before revision surgery, your surgeon needs to know where the old fat is. Ultrasound shows the depth of the grafts, any oil cysts and areas of scar tissue. It also helps to confirm that previous fat sits in the layer above the muscle. If you already have an ultrasound or MRI, send it with your enquiry. If not, we carry out an ultrasound assessment in Istanbul before surgery.
Records from your previous clinic
Operation notes are very useful. They show how much fat was moved, where it went and whether there were problems. Ask your previous clinic for a copy of your notes. In the UK and EU you have a legal right to your medical records. If you cannot get them, we can still plan with photos, an exam and ultrasound.
General health
You should be at a stable weight for at least six months. A healthy BMI lowers the risk of complications. You should stop smoking and all nicotine for four weeks before and after surgery. Health problems such as diabetes or high blood pressure must be well controlled. Our team will ask for a full medical history.
Who should not have a BBL revision
- Patients less than 12 months after their previous BBL, unless there is a medical reason to act sooner
- Patients with buttock implants or non-fat fillers, who need a different assessment
- Patients with very little donor fat who want a large increase in size
- Anyone who is pregnant, breastfeeding or has an uncontrolled health condition
- Anyone expecting a perfect or guaranteed result
If you have had illegal or unknown injections, such as silicone or other fillers, tell us. These need a specialist plan and are not treated with a standard BBL revision.
Revision after surgery at another clinic
Most of our revision patients had their first BBL somewhere else. That is fine. We do not judge past choices. We focus on your tissue as it is today and what can be done safely. If your first surgery was with us, we review your case under our aftercare policy and discuss options with you.
What should you send for a free BBL revision review?
A good plan starts with good information. When you contact us, please send:
- Clear photos from the front, back, both sides and at an angle, in good light
- Photos from before your first BBL, if you have them
- The date of your first surgery and the name of the clinic
- Operation notes, including the amount of fat moved, if available
- Any ultrasound or MRI reports
- A short note on what bothers you most
Your photos are handled in confidence and seen only by the medical team. Our surgeon replies with an honest view, a plan and a fixed price for your BBL revision.
How is BBL revision surgery done?
BBL revision surgery combines several techniques in one operation. The surgeon may reduce excess fat, smooth lumps, release scar bands, remove cysts and add new fat where volume is missing. Most revisions take 4–6 hours under general anaesthesia.
Step 1: assessment and marking
On the day before surgery, you have blood tests, an ECG and a meeting with the anaesthetist. The surgeon examines you standing up and lying down. Ultrasound maps the old grafts, cysts and scar tissue. Your body is then marked so each area has a clear plan.
Step 2: liposculpting of irregularities
Fine cannulas are used to smooth high spots, ridges and overfilled areas. In donor areas, the surgeon blends dents from the first liposuction. This step is gentle and precise, because over-treating can create new dents. It is often where the biggest visual change comes from.
Step 3: subcision of scar bands
Scar bands that pull the skin down are released with a thin blunt instrument. This frees the skin and creates space. Released areas can then be filled with small amounts of fat so the surface lies flat.
Step 4: removal of necrotic fat and oil cysts
Oil cysts found on ultrasound may be drained through a small needle or cannula. Firm areas of fat necrosis may be broken up or removed. Small cysts that cause no symptoms are sometimes left alone, because removing them may cause more scarring than they are worth.
Step 5: re-grafting with fresh fat
Where volume is needed, fat is taken from donor areas, purified and injected again. All fat is placed in the subcutaneous layer only, under the skin and above the muscle. We use ultrasound guidance so the surgeon can see the cannula tip during injection. Fat is placed in many small threads, not large pools, to improve survival.
Step 6: closing and compression
The small entry points are closed with fine stitches. A compression garment is fitted while you are still asleep. You wake in a recovery room with nursing staff close by. Most patients walk a few steps the same evening, which lowers the risk of blood clots.
How long does a BBL revision take?
Most revisions take 4–6 hours. Simple cases, such as smoothing a few irregular areas, are shorter. Complex cases with large cysts, heavy scar tissue and full re-grafting take longer. Your surgeon will give you a time estimate after the assessment.
Why does ultrasound matter more in a revision BBL?
In a first BBL, the layers under the skin are clean and easy to read. In a revision BBL, scar tissue can be tough and uneven. It can push a cannula off course. Real-time ultrasound lets the surgeon see the tip of the cannula and the muscle layer below. This helps keep every graft in the safe layer above the muscle. It also helps the surgeon find old cysts and avoid them during injection.
How should you prepare for a BBL revision?
Good preparation lowers risk and helps healing. Start about six weeks before your BBL revision date. Your coordinator sends a checklist, but these are the main steps.
Six to four weeks before
- Stop smoking, vaping and all nicotine products
- Keep your weight stable; avoid crash diets
- Tell us about all medicines, vitamins and supplements
- Book flights that allow the full 7–10 day stay
Two weeks before
- Stop blood-thinning supplements such as fish oil, vitamin E and ginkgo, unless your doctor says otherwise
- Avoid alcohol
- Arrange help at home for the first week after you return
- Pack loose clothes, slip-on shoes and a travel BBL pillow
The day before surgery
Eat a light meal and follow the fasting time given by the anaesthetist. Drink water as advised. Shower and do not apply body cream. Bring your past notes and scans to the surgeon visit. Ask any last questions before you sign the consent form. You should feel clear about the plan, the limits and the recovery.
What is BBL revision recovery like, week by week?
BBL revision recovery is similar to a first BBL but can feel slower. Expect swelling and bruising for several weeks. Most patients return to desk work in 2–3 weeks and see the final shape at six months or later.
| Time after surgery | What to expect | What you can do |
| Days 1–2 | Soreness, swelling and some fluid from entry points | Short walks every few hours; stay in hospital 1–2 nights |
| Days 3–7 | Bruising peaks, then starts to fade | Hotel rest, lymphatic massage, surgeon check |
| Week 2 | Swelling starts to ease; you feel more mobile | Fly home once cleared; light daily tasks |
| Weeks 2–3 | Energy returns; firm areas are common | Desk work, using a BBL pillow for sitting |
| Weeks 4–6 | Most bruising has gone; shape still swollen | Light exercise such as walking; sit normally when advised |
| Weeks 6–8 | Garment time reduces | Return to the gym, slowly building up |
| Months 3–6 | Fat settles and the shape becomes clearer | Normal life; video follow-ups |
| 6–12 months | Final result; scar tissue keeps softening | Final review of the outcome |
Sitting, sleeping and compression
Avoid sitting directly on the buttocks for the first weeks. Use a BBL pillow under your thighs when you must sit. Sleep on your front or side. Wear your compression garment day and night for about six weeks, unless your surgeon gives other advice. The garment helps reduce swelling and supports smooth healing.
Massage and aftercare
Lymphatic drainage massage helps move fluid and can soften firm areas. Our packages include several sessions in Istanbul. Massage should be gentle, and deep pressure on new grafts is avoided. After you fly home, you have 12 months of remote aftercare with video check-ups. For a general guide, read our BBL recovery timeline.
Why revision recovery can feel slower
Scar tissue does not drain fluid as well as healthy tissue. This means swelling can last longer and firm areas are more common. Be patient. Judging the result too early can lead to worry about a problem that will settle with time.
Is BBL revision safe? Risks you should know
A BBL revision is major surgery and carries real risks. With a trained surgeon, an accredited hospital and safe fat-injection technique, the risks can be reduced but never removed. Revision also carries some extra risks because of scar tissue.
Fat embolism
The most serious risk of any gluteal fat transfer is fat embolism. This happens when fat enters large veins in or under the buttock muscle and travels to the lungs. It can be fatal. Safety groups now advise that fat should only be placed under the skin, never into the muscle. The American Society of Plastic Surgeons and the British Association of Aesthetic Plastic Surgeons both publish advice on BBL safety.
At BBL Turkey, all fat is placed in the subcutaneous layer only. Our surgeons use real-time ultrasound to see where the cannula sits. In a revision, scar tissue can push the cannula in the wrong direction, so this guidance matters even more.
Seroma and infection
A seroma is a pocket of fluid under the skin. It may need draining with a needle. Infection is uncommon but can happen with any surgery. You receive antibiotics, and your coordinator will help you get checked quickly if you notice redness, heat or fever.
Blood clots (DVT and PE)
Long surgery and long flights both raise the risk of blood clots in the legs (DVT). A clot can travel to the lungs (PE), which is an emergency. We lower this risk with early walking, compression stockings and blood-thinning medication where suitable. We also advise you to wait before flying home. The NHS guidance on cosmetic surgery abroad is a useful read before you travel.
Fat resorption and lower fat survival
Some new fat will always be absorbed. In revision cases, fat survival can be lower than in a first BBL, because scar tissue has a weaker blood supply. Some patients need a further small procedure later to fine-tune the result.
Asymmetry and contour irregularity
Even after revision, the two sides may not match exactly. New dents or lumps can also appear as the fat settles. Many of these improve with time and massage. Some may need a minor touch-up after at least six to twelve months.
Scarring, numbness and skin changes
Entry scars are small, but some people form thicker scars. Areas of numbness are common and usually improve over months. Skin colour changes over treated areas are rare but possible.
What warning signs should you report after a BBL revision?
Contact your coordinator or seek urgent care if you notice any of these:
- Shortness of breath, chest pain or a fast heartbeat
- Pain, swelling or warmth in one calf
- Fever, chills or spreading redness
- A sudden, painful swelling on one side
- Pus or a bad smell from an entry point
Our coordinator is available 24/7, in Istanbul and after you fly home. For breathing problems or chest pain, call your local emergency number first.

BBL revision before and after: what results to expect
BBL revision before and after photos usually show a smoother, more balanced shape rather than a dramatic jump in size. The main change is often in the outline: a softer hip line, an even pair of buttocks and fewer dents or lumps.
What a realistic change looks like
In a revision BBL, the "before" photo already shows surgery. So the "after" photo shows correction, not a first transformation. Typical changes include:
- A shelf or high ledge reduced into a rounder curve
- Two sides that look closer in size and height
- Hip dips filled for a smoother side view
- Fewer visible dents in the back, flanks or thighs
- Lumps from oil cysts reduced or removed
- A modest rise in volume where fat had been lost
When you will see the final result
Early photos show a lot of swelling. The shape can look larger or more uneven at first. The real BBL revision before and after picture appears at around six months, and it can keep improving up to a year. Judge the result at that point, not in the first weeks.
How long the results last
Fat that survives the first few months behaves like normal body fat. It stays as long as your weight stays stable. Gaining or losing a lot of weight will change the shape. Pregnancy and ageing also play a part.
To protect your result, keep a steady weight, eat well and stay active once you are cleared. Avoid large swings in weight. If you plan a pregnancy, tell your surgeon, as it can change the shape of the hips and buttocks.
Viewing real cases
You can see real patient results in our BBL before and after gallery. When you look at any gallery, check the lighting, the pose and the time since surgery. Results vary between people, and no clinic can promise a specific outcome.
How to read BBL revision before and after photos
Good photos are honest photos. Look for the same pose, the same light and the same camera height in both images. Check the date of the "after" photo. A result at two weeks is mostly swelling. A result at six to twelve months is a fair test. Look at the side view, not just the back. The side shows the hip line, the shelf and the upper curve best.
What can a revision BBL not do?
A revision cannot fully undo the first surgery. It cannot make two sides exactly the same. It cannot add large volume if there is little donor fat. It cannot remove every lump or every scar. Knowing these limits before surgery is the best way to be happy with the result.
How much does a BBL revision cost in Turkey?
BBL revision cost in Turkey starts from $5,990 for an all-inclusive package. This covers surgery, hospital stay, a 5-star hotel, VIP transfers and aftercare. A comparable revision often costs £14,000–£20,000 in the UK or $18,000–$28,000 in the US, before travel.
BBL revision Turkey package tiers
| Package | Price | Hospital nights | Hotel nights | Massage sessions |
| Standard | $5,990 | 1 | 5 | 4 |
| Premium (most popular) | $6,990 | 2 | 7 | 7 |
| Ultimate | $8,490 | 2 (private room) | 9 (suite, companion included) | 12 |
Premium adds a BBL pillow, a personal translator and a private vehicle. Ultimate adds a follow-up ultrasound, a suite for you and a companion, and a scar-care kit.
What affects the BBL revision cost
Revisions vary more than first BBLs. The final BBL revision cost depends on how many problems need treating, the amount of scar tissue, the number of cysts and whether fresh fat is needed. Your fixed price is confirmed after the surgeon reviews your photos and records. There are no hidden extras after that.
What is not included
Flights are not included. Travel insurance, extra hotel nights and meals outside breakfast are also extra. If you want a longer stay or added comfort, you can add these when you book. A 30% deposit secures your surgery date.
Compare prices with 16 countries
The interactive price comparison engine on this page compares Turkey with 16 European and North American countries. It shows typical surgery prices next to the Turkey package so you can see the full picture. For all of our prices in one place, visit BBL Turkey prices.
Is a cheaper BBL revision worth the risk?
Price matters, but it should never be the first question. A revision is harder than a first BBL. Low prices can mean less time in theatre, no ultrasound guidance or a surgeon you never meet. Ask what the price includes and who will operate. A fair BBL revision Turkey package should name the surgeon, the hospital and the aftercare in writing.
How do you choose a surgeon for a revision BBL?
Choose a surgeon who does revision work often, uses subcutaneous-only, ultrasound-guided fat injection and operates in an accredited hospital. They should review your records, explain the limits clearly and offer follow-up after you go home.
Questions to ask
- How many BBL revisions do you perform each year?
- Will you personally perform my surgery?
- Do you use ultrasound to guide fat injection?
- Where will the fat be placed, and in which layer?
- What can and cannot be corrected in my case?
- What happens if I need a touch-up later?
Credentials that matter
Look for membership of bodies such as the International Society of Aesthetic Plastic Surgery (ISAPS). Check that the hospital is accredited and has an intensive care unit. Ask to see revision cases, not just first BBLs. Our guide on how to choose a BBL surgeon in Turkey covers this in more depth.
Read reviews from patients who had revision surgery, not only first BBLs. Ask how the clinic handles problems after you fly home. A good clinic gives you a named contact and a clear plan for follow-up care.

Can a failed BBL be fixed, and what are the alternatives?
In most cases a failed BBL can be clearly improved, but not always made perfect. Depending on the problem, a full revision may not be needed. Smaller fixes or a different procedure can sometimes give a better balance of benefit and risk.
| Option | Best for | Adds volume? | Typical surgery time |
| Full BBL revision | Several problems at once: shape, volume and lumps | Yes, if donor fat allows | 4–6 hours |
| Liposculpting only | Overfilling, shelf look or donor-site dents | No | 2–3 hours |
| BBL with hip dip correction | Poor hip transition with good buttock shape | Yes, at the hips | 3–4 hours |
| 360 liposuction | Waist and back contour without buttock change | No | 2–3 hours |
| Mommy makeover | Shape change after pregnancy, with loose tummy skin | Depends on plan | Varies |
When to do nothing
Sometimes the best advice is to wait or to leave a result alone. Small irregularities may soften with time. If the risk of more surgery is higher than the likely gain, our surgeon will tell you. A free review costs nothing and does not commit you to surgery.
Can you have more than one revision?
Yes, but each extra operation adds scar tissue and lowers fat survival. Most surgeons prefer to wait at least a year between procedures. A careful first revision with clear goals gives the best chance of avoiding a third operation.
