What is a lipo 360 BBL?
A lipo 360 BBL is two procedures done in one session. The first is 360 liposuction, which slims the whole trunk. The second is a Brazilian Butt Lift, which uses that fat to shape the buttocks and hips. You get a smaller waist and fuller curves at the same time.
The word "360" means the surgeon works all the way around your body. Fat is taken from the front, the sides and the back. This is different from basic liposuction, which may treat just the tummy. Because the waist is sculpted from every angle, the new curves look balanced from the side and from behind.
Which areas are treated?
Most plans treat a core set of areas. Your surgeon may add more if you have spare fat and the extra volume helps your shape.
- Core areas: upper and lower abdomen, flanks (love handles), lower back and waist
- Common add-ons: upper back and bra-line rolls, arms and inner thighs
- Fat transfer areas: the buttocks and the outer hips, and sometimes the hip dips
Most patients receive 800–1,500cc of purified fat per side. The exact amount depends on how much fat you have and the shape you want. A surgeon will never take more fat than is safe in one session.
Why patients search for 360 lipo and BBL together
Many patients search for 360 lipo and BBL as one package for a simple reason. Each part makes the other look better. A slimmer waist makes the buttocks look rounder. Fuller hips make the waist look smaller. The two steps together change your waist-to-hip ratio more than either step alone.
You may see this surgery called by different names. Clinics use "BBL 360 lipo", "360 lipo BBL" and "BBL and lipo 360" for the same idea. Some also call it a "BBL 360". At BBL Turkey, we call it the lipo 360 BBL. Whatever the name, ask each clinic exactly which areas they treat. Some "360" quotes leave out the back.
How is it different from a standard BBL?
A standard BBL takes fat from one or two areas, often the tummy and flanks. This surgery treats the full circle of the trunk. That gives the surgeon more fat to work with. It also gives a sharper waist, because the lower back and sides are thinned as well.
The trade-off is a longer operation and a slightly longer recovery. There are more small incisions and more areas to heal. For most patients with enough fat, the gain in shape is worth it. Your surgeon will tell you honestly if a smaller plan would suit you better.
What can a lipo 360 BBL not do?
It is not a weight-loss operation. It works best on stubborn fat that diet and exercise have not shifted. It does not remove loose skin. It also does not change your bone structure, so your natural hip width sets the limits of your shape. If you have a lot of loose skin on the tummy, a tummy tuck may be needed. Our Mommy Makeover combines both.
Who is a good candidate for a BBL and lipo 360?
A good candidate for a BBL and lipo 360 is healthy, at a stable weight and has enough spare fat to move. Most suitable patients have a BMI of roughly 22 to 32. A surgeon checks every case before you book.
You may be a good fit if you
- Have a BMI of about 22–32, with fat on the tummy, flanks or back
- Have kept a stable weight for at least six months
- Do not smoke or vape, or can stop all nicotine for four weeks before and after surgery
- Are in good health, with no uncontrolled heart, lung or blood conditions
- Have good skin tone around the waist
- Can avoid sitting on your buttocks for the first two weeks
- Want a natural result and understand that some fat is always reabsorbed
When a BBL and lipo 360 is not right
Some people should not have this surgery, or should wait. Being honest about this is part of safe care.
- Very low body fat: if your BMI is under 22, there may not be enough fat. A Skinny BBL is designed for slim patients.
- A BMI well above 32: risks rise and fat survival can fall. Losing weight first is usually safer.
- Pregnancy: you should not be pregnant, breastfeeding or planning a baby in the next year.
- Clotting problems: a past clot, a known clotting disorder or some hormone medicines need specialist review.
- Uncontrolled illness: such as diabetes, heart disease or high blood pressure that is not managed.
- Unrealistic goals: no surgeon can promise an exact size or copy a photo of someone else.
If you are unsure, send us your photos. One of our surgeons will look at your case and tell you which option fits your body. You can also compare options on our 360 liposuction page if you want contouring without a fat transfer.
Can you have a lipo 360 BBL after pregnancy or weight loss?
Yes, many people do. The key questions are how much loose skin you have and whether your weight is stable. Your surgeon will look at both in your photos.
After pregnancy
Wait at least six months after giving birth, and until you have stopped breastfeeding. Your body needs time to settle. If your tummy skin has stayed firm, liposuction alone may give a good result. If you have loose skin or a split in the tummy muscles, liposuction will not fix it. In that case a tummy tuck works better, often as part of a Mommy Makeover.
After major weight loss
Your weight should be stable for at least six months first. After a big loss, skin is often looser and fat can be thinner. That can limit how much fat is available to move. It can also make the skin look uneven after liposuction. Some patients do better with a staged plan, with skin removal first and fat transfer later.
Weight-loss medicines are now common. If you take one, tell your surgeon. You may need to stop it before surgery, and your weight should be steady before you book.
How is 360 lipo and BBL surgery done?
360 lipo and BBL surgery is done in one session under general anaesthesia and takes 4–6 hours. The surgeon removes fat around the trunk, cleans it, then places it into the buttocks and hips. The steps below are what happens on the day.
Before surgery: planning and tests
Planning starts weeks before you fly. You send photos, your height, weight and health history. A surgeon reviews them and sends you a written plan. It lists the areas to treat, the expected fat volume and a fixed price.
When you arrive in Istanbul, you have blood tests and a heart tracing (ECG). An anaesthetist checks you are fit for a long operation. You then meet your surgeon face to face. You talk about shape, size and your goals. You can still change your mind at this stage.
Step 1: Marking while you stand
Your surgeon draws on your skin while you stand up. Fat sits differently when you lie down, so marking standing up is more accurate. The marks show where fat will come out and where it will go in.
Step 2: General anaesthesia and safety checks
A consultant anaesthetist puts you to sleep and watches you the whole time. You wear calf pumps to keep blood moving in your legs. You also get antibiotics. These steps lower the risk of clots and infection.
Step 3: 360 liposuction
The surgeon first fills the fat layer with a fluid called tumescent solution. It numbs the area and reduces bleeding and bruising. Then a thin tube, called a cannula, removes fat through small cuts of 3–4mm. These cuts are hidden in skin creases where possible.
The surgeon works on the tummy, flanks, waist and lower back. You are turned during surgery so the back can be treated as well as the front. The aim is a smooth, even curve, not just a flat tummy. Power-assisted or VASER tools may be used, depending on your tissue.
Step 4: Cleaning the fat
Not all of the fat that comes out is used. It is processed in a closed, sterile system. Blood, fluid and oil are separated from healthy fat cells. Only clean fat is kept for the transfer. Good processing helps more of the fat survive.
Step 5: Safe fat transfer to the buttocks and hips
This is the step that matters most for safety. Our surgeons place fat only in the layer under the skin, called the subcutaneous layer. They never inject into or under the buttock muscle. They use blunt cannulas and real-time ultrasound to see exactly where the tip is.
Fat is laid down in many thin threads, not in large pools. This gives each droplet a better blood supply, which helps it survive. The surgeon shapes the upper buttocks, the lower curve and the outer hips. Hip dips can be filled in the same step.
Step 6: Closing and compression
The small cuts are closed with fine stitches or left to drain, depending on the surgeon's method. You are dressed and fitted with a compression garment before you wake up. You then go to the recovery room, where nurses check you closely.
How long does it take and who is in the room?
The full operation usually lasts 4–6 hours. Larger plans, with arms or thighs added, take longer. Your team includes your surgeon, an assistant surgeon, a consultant anaesthetist and theatre nurses. Every case takes place in a JCI-accredited hospital with intensive care on site.
How should you prepare for surgery?
Start preparing at least a month before you fly. Good preparation makes recovery easier, and most of it is simple and in your control.
- Stop nicotine at least four weeks before surgery. This includes vapes, patches and gum. Nicotine reduces blood flow and can cause fat loss and wound problems.
- Keep your weight stable. Do not crash diet. Your surgeon needs your normal fat to work with.
- Check your medicines. Some pills, such as aspirin, certain supplements and the combined contraceptive pill, may need to stop. Your surgeon will tell you which ones.
- Eat well and drink water. Protein and iron help healing.
- Plan your seating. Buy or bring a BBL pillow for the flight home and for work.
- Arrange help at home for the first week after you return.
Our BBL Turkey travel guide covers flights, what to pack and how to plan your time in Istanbul.
What does a lipo 360 BBL Turkey trip look like, day by day?
Most lipo 360 BBL Turkey trips last 7–10 days. Here is a typical plan. Your coordinator will adjust it to your flights and your recovery.
- Day 1, arrival: a driver meets you at the airport and takes you to your 5-star hotel. Your coordinator calls to go through the plan.
- Day 2, tests and consultation: you have blood tests, an ECG and an anaesthetist check. You meet your surgeon, agree your shape and are marked.
- Day 2 or 3, surgery: you have your operation and stay in hospital for one or two nights.
- Days 4–6, hotel rest: you rest at the hotel, take short walks and start lymphatic drainage massage.
- Days 6–9, checks: your surgeon reviews your wounds and garment fit. More massages help ease swelling.
- Days 7–10, fly home: once cleared, you fly home with your garments, medicines and written aftercare plan.
It helps to bring a companion. Our Ultimate package includes a hotel suite where a companion can stay with you. If you travel alone, your coordinator is on call day and night.
What to pack
- Loose, front-opening clothes and slip-on shoes
- A BBL pillow for the flight home, if you are not on a package that includes one
- Flight socks, a refillable water bottle and lip balm
- A phone charger, a list of your medicines and your travel insurance details
What is 360 lipo BBL recovery like, week by week?
360 lipo BBL recovery is steady but takes patience. Most people walk the same day, fly home after 7–10 days and go back to desk work in 2–3 weeks. Your final shape shows at 3–6 months. Our full BBL recovery timeline goes into more detail.
| Time after surgery | What to expect | What you can do |
| Days 1–2 | Swelling, soreness, fluid leaking from small cuts | Rest in hospital, short walks with a nurse |
| Days 3–7 | Bruising peaks, tightness around the waist | Hotel rest, daily walks, first massages |
| Days 7–10 | Pain eases, garment feels more comfortable | Final check with your surgeon, fly home |
| Weeks 2–3 | Bruising fades, energy returns | Desk work, light housework, no direct sitting |
| Weeks 3–6 | Swelling keeps going down | Sit on a BBL pillow, longer walks |
| Weeks 6–8 | Waist looks more defined | Return to the gym once cleared |
| Months 3–6 | Fat has settled, final shape appears | Normal life, keep weight stable |
The first 48 hours
You stay in hospital for one or two nights. Nurses give you pain relief and fluids through a drip. You will feel sore and swollen. Pink fluid may leak from the small cuts. This is normal and shows the tumescent fluid draining out. You are helped to stand and walk within hours. Walking early helps prevent blood clots.
Week 1: hotel recovery in Istanbul
You move to your 5-star hotel. Your coordinator checks on you every day. You start lymphatic drainage massage, which helps move swelling. Wear your compression garment about 23 hours a day. Sleep on your front or side. Do not sit directly on your buttocks, except for short toilet breaks.
Your surgeon sees you before you fly. They check your wounds, your garment fit and your general health. Only then will they clear you to travel home.
Weeks 2–6: getting back to normal
Most people return to desk work after 2–3 weeks. You may still feel tired by the end of the day. From week three you can sit with a BBL pillow under your thighs. Bruising fades and the waist starts to show. Keep walking each day, but avoid heavy lifting, saunas and swimming.
Weeks 6–12: exercise and shape
Around week six, most surgeons allow a slow return to the gym. Start with cardio and upper body work. Add squats and leg work later. Normal sitting is usually fine by now. Some areas may feel firm or numb for a while. This slowly improves.
Months 3–6: final results
By three months, the fat that has survived has its own blood supply. It is now permanent. Swelling in the lower back and tummy can take up to six months to settle fully. This is when your true result shows. We ask for photos at this point to review your outcome.
Tips for a smoother recovery
- Walk a little every hour while awake
- Drink plenty of water and eat enough protein
- Wear your garment exactly as advised
- Do not smoke or vape for at least four weeks
- Do not put pressure on the buttocks for the first two weeks
- Report fever, chest pain, breathlessness or calf pain straight away
Why do compression garments and massage matter?
Garments and massage control swelling and help your skin settle smoothly over the new shape. They are part of your treatment, not an optional extra.
Compression garments
A compression garment, often called a faja, presses gently on the treated areas. It cuts down swelling and helps the skin shrink back evenly. It also supports your waist while the tissues heal. Most surgeons ask you to wear it about 23 hours a day for the first few weeks. After that, you wear it for fewer hours for a few more weeks. Your surgeon will tell you when you can stop.
The garment should feel firm but not painful. It should not dig in or press hard on your buttocks. Tell your coordinator if it rubs or feels too tight.
Lymphatic drainage massage
This is a light, gentle massage. It helps your body move fluid away from swollen areas. It can also ease hard patches that may form under the skin after liposuction. Your package includes sessions in Istanbul. Many patients book more sessions when they get home. The massage should be done by a trained therapist who knows about BBL aftercare.
Aftercare when you are home
Your care does not end at the airport. You get 12 months of remote aftercare, with video calls and photo reviews. Your coordinator is there to answer questions and pass them to your surgeon.
Is a lipo 360 BBL safe? Risks you should know
This is major surgery, and all surgery carries risk. Most serious risks can be reduced by the right technique, the right hospital and the right patient. No clinic can remove risk completely, and you should be wary of any that says it can.
Fat embolism: the most serious risk of any BBL
Fat embolism happens when fat enters a large vein and travels to the heart or lungs. With gluteal fat transfer, this has mainly been linked to fat being placed in or under the buttock muscle, where the large veins run. It is rare, but it can be fatal.
This is why safety bodies now advise placing fat only above the muscle. The British Association of Aesthetic Plastic Surgeons has issued guidance on BBL safety, and the American Society of Plastic Surgeons explains the same principle for patients. Our surgeons follow it on every case. They inject only into the subcutaneous layer, with blunt cannulas and real-time ultrasound to confirm the depth.
Other risks
- Seroma: a pocket of fluid under the skin, most often in the lower back. It is usually drained in clinic.
- Infection: at a cut or in the fat. Sterile technique and antibiotics lower the risk.
- Blood clots (DVT and PE): a clot in the leg can travel to the lungs. Calf pumps, early walking, compression and, where needed, blood thinners help prevent this. Long flights add risk, so we time your return with care.
- Asymmetry: one side may settle slightly differently. Small differences are normal. Larger ones may need a touch-up.
- Fat resorption: some of the moved fat is always reabsorbed by the body. The amount varies from person to person.
- Contour irregularity: dents, ripples or lumps can appear after liposuction, especially if skin tone is poor.
- Other effects: numbness, oil cysts, scarring at the small cuts, skin colour change and long-lasting swelling.
How we reduce your risk
Safety starts with saying no to the wrong patient. We decline cases with a high BMI, uncontrolled illness or unsafe expectations. We cap fat volumes and operating time. We use JCI-accredited hospitals with a consultant anaesthetist present throughout. You also get a 24/7 English-speaking coordinator and 12 months of remote aftercare.
The NHS has helpful advice on cosmetic surgery abroad. We suggest every patient reads it. Our own article, Is a BBL in Turkey safe?, explains what to check before you book any clinic.
Results vary between people and no outcome can be guaranteed. Your surgeon will go through every risk with you before you agree to surgery.
Which warning signs need urgent help after surgery?
Most recovery is slow and steady, but some signs need help straight away. Do not wait to see if they pass.
- Chest pain, a fast heartbeat or sudden breathlessness: call emergency services at once.
- A hot, swollen or painful calf: this can be a sign of a blood clot.
- Fever, chills or spreading redness: these can be signs of infection.
- A sudden, painful swelling on one side: this can mean a fluid or blood collection.
- Feeling confused, faint or very unwell: get medical help quickly.
In Istanbul, call your coordinator day or night. At home, call your local emergency number first, then tell our team.
What results can you expect, and how long do they last?
A lipo 360 BBL gives a slimmer waist and a rounder, fuller bottom and hips. The fat that survives after three to six months is permanent. It will last for many years if your weight stays stable.
BBL 360 lipo results month by month
Your BBL 360 lipo results change a lot over the first few months. Right after surgery, you will look larger than your final size. That is swelling plus extra fat that the body will partly reabsorb. Over the first six weeks, the buttocks soften and drop slightly into a more natural shape.
The waist often looks better each month as swelling leaves the tummy and back. By three months, most people can see their new outline clearly. By six months, the result is close to final. You can view real cases, sorted by body type, in our BBL before and after gallery.
What affects how much fat survives?
Fat survival is never 100%. Some fat is always lost, and the amount varies. These factors make a difference:
- Technique: gentle harvesting, clean processing and thin, even layers help fat survive.
- Pressure: sitting or lying on the buttocks too early can squash new fat and cut off its blood supply.
- Nicotine: smoking and vaping reduce blood flow and can lower survival.
- Weight changes: large weight loss shrinks transferred fat just like other fat.
- Your body: skin quality, blood supply and genetics also play a part.
How to keep your results
Transferred fat behaves like the fat it came from. If you gain weight, it can grow. If you lose a lot of weight, it can shrink. Keep your weight within a few kilos of your weight on the day of surgery. Eat well, stay active and avoid crash diets. Pregnancy and ageing will change your shape over time, as they would without surgery.
If your shape changes years later, or you were unhappy with surgery elsewhere, a BBL revision can sometimes help.

Why is a lipo 360 BBL in Turkey cheaper?
Lower prices in Turkey come from lower running costs, not lower standards. Staff wages, rent and hospital fees cost less in Istanbul. Surgeons also perform these operations often, which makes the process efficient. The hospitals we use hold the same JCI accreditation used to judge leading hospitals in the US and Europe.
A low price should never be the only reason to choose a clinic. Be careful with offers that are far below the market. Ask what is left out, who does the surgery and which hospital you will be in.
How do you choose a surgeon for a lipo 360 BBL?
Choose a qualified plastic surgeon who does this exact operation often, works in an accredited hospital and uses ultrasound-guided, subcutaneous-only fat transfer. Experience and safety habits matter more than price.
Questions to ask any clinic
- Who will do my surgery, and can I see their qualifications?
- Are they a member of a recognised body such as ISAPS?
- Where will fat be placed, and do you use ultrasound to check the depth?
- Which hospital will I be in, and is it accredited?
- Will a consultant anaesthetist be with me the whole time?
- How much fat do you plan to move, and why?
- What happens if I have a problem after I fly home?
Our guide on how to choose a BBL surgeon in Turkey covers these questions in more depth.
Who are the surgeons at BBL Turkey?
Your operation is planned and performed by one of the specialist plastic, reconstructive and aesthetic surgeons on our surgical team in Istanbul. You can read each surgeon's training and qualifications on their profile, and you meet your surgeon on a video call before you book and in person before surgery. We have three senior surgeons who share one set of safety rules. Your surgery is planned and performed by one of our senior surgeons. You can read more about the whole team on our surgeons page.

Istanbul is about a four-hour flight from London. A 7–10 day trip is enough for most patients. That makes it easier to fit surgery around work and family.
Lipo 360 BBL vs other options: which is right for you?
This surgery suits people with moderate spare fat who want both a smaller waist and fuller curves. Other options may suit you better if you are very slim, only want to fix hip dips or need skin removal.
| Procedure | Best for | Fat transfer | Main difference |
| Lipo 360 BBL | BMI about 22–32, wants waist and curves | Yes, buttocks and hips | Full trunk contouring plus a fuller bottom |
| Skinny BBL | Slim patients with little fat | Yes, smaller volumes | Fat taken from many small areas |
| BBL + Hip Dips | Patients with hollow hips | Yes, with focus on hips | Extra focus on the side profile |
| 360 liposuction | Wants a slimmer waist only | No | No change to buttock size |
| Mommy Makeover | Loose skin after pregnancy | Optional | Adds a tummy tuck and often breast surgery |
Lipo 360 BBL vs buttock implants
Implants add size without moving fat. They do not slim the waist, and they can shift, harden or need replacing. Fat transfer uses your own tissue, so it looks and feels more natural. Implants may suit very slim people who have almost no fat to spare.
Lipo 360 BBL vs non-surgical options
Fat-freezing and skin-tightening devices can reduce small bulges. They cannot add volume to the buttocks. Fillers in the buttocks are not advised by many surgeons and can carry serious risks. For a real change in shape, surgery is usually the only option. The NHS page on liposuction explains what fat removal can and cannot do.
